Nutrigenomics has had a strange decade. It was overhyped, then quietly dismissed, then quietly proven, and is now back in mainstream practice — but with a much clearer evidence base. So before you spend several thousand rand on a DNA test, here's what's actually true and what's still marketing.

What is nutrigenomics, exactly?

Nutrigenomics is the study of how your individual genetic variations influence the way your body processes nutrients. Some people metabolise caffeine quickly; others slowly. Some convert beta-carotene to vitamin A efficiently; others poorly. Some carry variants that increase the risk of blood lipid disturbance on a high-saturated-fat diet; others tolerate it well.

These differences are encoded in single-nucleotide polymorphisms (SNPs) — single letter variations in genes that influence enzyme activity, transporter function, or receptor sensitivity. A nutrigenomic test reads a curated set of these SNPs and produces a profile.

What can it actually tell you?

The areas where the evidence is genuinely strong:

  • Caffeine metabolism (CYP1A2 variants) — slow metabolisers experience the cardiovascular effects of caffeine for hours longer
  • Lactose tolerance (LCT/MCM6 variants) — clarifies whether intolerance is genetic or temporary
  • Folate processing (MTHFR variants) — informs choice between folic acid vs. methylated folate
  • Saturated fat sensitivity (APOE variants) — meaningful guidance on dietary fat composition
  • Vitamin D conversion — many South Africans are sub-optimal; some need higher intake to reach the same blood level
  • Iron loading risk (HFE variants) — important for haemochromatosis screening
  • Inflammatory response patterns — informs anti-inflammatory dietary emphasis

What it cannot tell you

Your DNA does not contain a meal plan. It contains a profile of biochemical tendencies.

Be cautious of any nutrigenomic report that:

  • Tells you what to eat for breakfast
  • Predicts your weight loss to the kilogram
  • Claims to identify "your perfect diet"
  • Talks about "DNA-coded recipes"
  • Doesn't reference the specific genes and SNPs analysed

These framings overstate what the science actually delivers. A genetic profile gives us probabilities and tendencies — not deterministic answers. It informs the diet, it doesn't dictate it. Your environment, lifestyle, gut microbiome, training history, and current health status still account for the majority of nutrition outcomes.

Why I use 3x4 Genetics

Is it worth it for you?

The patients who get the most value from nutrigenomics:

  • People who feel they've tried the "right" diets without lasting result
  • Patients with a strong family history of cardiovascular disease, type 2 diabetes, or specific cancers — where genetic information meaningfully shifts the prevention plan
  • Athletes optimising performance and recovery
  • People with stubborn weight or inflammation issues who suspect there's a metabolic piece they haven't addressed
  • Patients with chronic micronutrient issues (low B12, persistent low vitamin D) who want to understand the underlying mechanism

Patients who probably don't need it:

  • Anyone with a clear, unaddressed lifestyle issue (poor sleep, no exercise, very high alcohol intake) — fix that first
  • Patients looking for a magic bullet — the test is a tool, not a transformation
Genetics load the gun; lifestyle pulls the trigger. A nutrigenomic test tells you which gun you're holding — it doesn't change what you choose to do.

The bottom line

Nutrigenomics is no longer fringe science. The clinical evidence in specific, well-defined pathways is solid, and the technology is now affordable and accessible in South Africa. Used well — by a practitioner trained in interpretation, with appropriate humility about what the data can and can't predict — it's one of the more powerful tools in modern dietetics.

Used badly — as a marketing gimmick attached to vague meal plans — it's an expensive disappointment. Choose carefully.
